Timely recognition and targeted interventions are essential to optimize functional recovery and prevent complications.

The corticospinal tract arises from Betz cells in the precentral gyrus, traversing the internal capsule, cerebral peduncles, pons, and medullary pyramids before decussating and descending in the lateral spinal cord. Lesions at any point yield UMN signs below the level of injury. Loss of inhibitory modulation on spinal reflex arcs leads to hyperreflexia, increased tone, and spasticity. Over time, disuse and altered muscle metabolism may result in mild disuse atrophy.

Clinical Manifestations

1. Muscle Weakness and Distribution

UMN lesions produce variable weakness patterns:
– Monoplegia/Hemiplegia/Paraplegia/Quadriplegia depending on lesion location and extent.
– Distal Muscles Predominantly Affected: Hand intrinsics, wrist extensors, and ankle dorsiflexors often show marked weakness.

2. Hypertonia and Spasticity

  • Velocity-Dependent Increase in Tone: Resistance to passive stretch that increases with speed.
  • Clasp-Knife Phenomenon: Sudden give after initial resistance due to Golgi tendon organ activation.

3. Reflex Changes

  • Hyperreflexia: Exaggerated deep tendon reflexes, often with spread to adjacent muscle groups.
  • Clonus: Sustained rhythmic oscillations, commonly at the ankle or wrist.
  • Pathological Reflexes: Release of primitive reflexes (Babinski, Oppenheim, Hoffman, Chaddock signs).

4. Preservation of Muscle Bulk

  • Minimal Early Atrophy: Trophic support from intact LMN prevents rapid wasting; chronic disuse may later contribute.

Diagnostic Workup

  1. Clinical Examination: Detailed neurologic assessment—muscle strength grading, spasticity scales (Modified Ashworth), reflex testing, and assessment of functional mobility.
  2. Neuroimaging: MRI of the brain and/or spinal cord to localize lesions, evaluate extent, and guide treatment planning.
  3. Electrophysiology: Motor evoked potentials (MEPs) assess corticospinal integrity; EMG distinguishes UMN from LMN involvement.
  4. Adjunctive Tests: Laboratory studies to identify inflammatory, infectious, or metabolic etiologies when indicated.

Differential Diagnosis

  • Lower Motor Neuron Lesions (e.g., peripheral neuropathy, anterior horn cell disease): Marked muscle wasting, fasciculations, diminished reflexes.
  • Extrapyramidal Disorders (e.g., Parkinsonism): Rigidity, bradykinesia, and tremor rather than spastic tone.
  • Cerebellar Lesions: Ataxia and dysmetria without spasticity or hyperreflexia.

Management Strategies

Acute Phase

  • Address Underlying Cause: Thrombolysis or thrombectomy for ischemic stroke; surgical decompression for compressive lesions.
  • Prevent Secondary Complications: Pressure ulcer prophylaxis, deep vein thrombosis prevention, and pulmonary hygiene.

Spasticity Control

  • Physical Therapy: Range-of-motion exercises, positioning, and splinting to maintain joint mobility and prevent contractures.
  • Pharmacotherapy: Oral agents (baclofen, tizanidine, dantrolene) titrated to balance tone reduction and muscle strength. Intrathecal baclofen pumps for refractory cases.
  • Botulinum Toxin Injections: Focal spasticity management in targeted muscle groups to improve function and ease caregiving.

Rehabilitation and Functional Recovery

  • Task-oriented Training: Gait training, repetitive upper limb exercises, and activities of daily living (ADL) practice to harness neuroplasticity.
  • Neuromodulation: Functional electrical stimulation (FES) and transcranial magnetic stimulation (TMS) as adjuncts to enhance motor relearning.
  • Adaptive Equipment: Orthoses (ankle-foot orthosis), wheelchair seating, and assistive devices to maximize independence.

Long-Term Considerations

  • Bone Health: Weight-bearing activities and osteoporosis screening to prevent fragility fractures in paralyzed limbs.
  • Psychosocial Support: Address mood disorders, caregiver burden, and community reintegration through multidisciplinary teams.

Prognosis

Outcome depends on etiology, lesion severity, and timeliness of intervention. Early, intensive rehabilitation improves functional gains. Chronic lesions may stabilize, but residual weakness and spasticity often persist.
