Pathophysiology

Muscle homeostasis relies on the balance between anabolic pathways (e.g., IGF-1/Akt/mTOR) and catabolic processes (e.g., ubiquitin-proteasome, autophagy). In atrophy, increased expression of muscle-specific E3 ubiquitin ligases (MuRF1, atrogin-1) accelerates protein degradation. Contributing factors include oxidative stress, inflammatory cytokines (TNF-α, IL-6), hormonal alterations (testosterone decline), and denervation-induced loss of trophic signaling.

Clinical Presentation

  • Weakness: Insidious in myogenic and disuse types; rapid in neurogenic atrophy.
  • Muscle Bulk Loss: Visible thinning, particularly in hands, calves, or quadriceps.
  • Functional Impairment: Difficulty rising from chairs, climbing stairs, or performing fine motor tasks.
  • Associated Symptoms: Fasciculations in neurogenic forms; myalgia in inflammatory myopathies; joint pain in reflex atrophy.

Diagnostic Evaluation

Management Strategies

  • Nutritional Support: Adequate protein intake (1.2–1.5 g/kg/day), vitamin D optimization, and amino acid supplementation (e.g., leucine) to enhance muscle protein synthesis.
  • Neuromuscular Stimulation: Electrical muscle stimulation in immobile or bedridden patients to attenuate disuse atrophy.

Pharmacologic Approaches

  • Anti-Catabolic Agents: Investigational use of myostatin inhibitors to reduce protein breakdown.
  • Anabolic Therapies: Testosterone or selective androgen receptor modulators (SARMs) in select hypogonadal or osteoporotic populations.
  • Anti-Inflammatory Treatments: Corticosteroids or immunosuppressive agents in inflammatory myopathies; monitor for steroid-induced myopathy.

Addressing Underlying Causes

  • Neurogenic Atrophy: Optimize management of neuropathies (e.g., glycemic control in diabetic neuropathy).
  • Reflex Atrophy: Treat joint pathology and pain; physical therapy to restore normal movement patterns.

Rehabilitation and Prevention

  • Early Mobilization: Initiate passive and active range-of-motion exercises post-surgery or during hospitalization.
  • Exercise Adherence: Multidisciplinary support, including physiotherapists and occupational therapists, to encourage adherence and functional recovery.
  • Fall Prevention and Safety: Balance training, home modifications, and assistive devices to reduce injury risk in frail populations.
